A very important consideration is the mental health of the patient.  The surgeon needs to make sure that the patient is not just a surgery junkie who is never satisfied with the outcome.  Some people are perfectionists and no outcome is going to satisfy them.  The surgeons need to make sure that the patient is psychologically balanced and has realistic expectations when it comes to the surgery.  The patient and the surgeon also need to ensure that the problem is fixable.  There is no point in doing additional surgeries if the problem cannot be fixed.
